    Comprehensive Initial Evaluation

    This is a 60-minute virtual appointment, where I perform a comprehensive review of your medical and psychiatric history with you. We explore your symptoms and discuss goals of treatment. Review the indications for recommended medications, potential benefits and side effects. We also review expected lab work that would need to be done before starting medications.

    Follow up

    This is a 30-minute virtual appointment, for medication management. I review your progress, and I assess if there are any side-effects of the medication. I review if any medication changes need to be made.

Self-Pay Rates

Comprehensive Initial Evaluation- $400

Follow Up - $250

Clients working with us as an out-of-network provider may request a superbill for submission to their insurance company for potential reimbursement. Coverage varies by plan, so we encourage clients to contact their insurance provider directly to understand their out-of-network mental health benefits.

What would working together look like?

Once you fill out a patient intake, I will contact you to discuss. If you choose to move forward, all new client forms, consents and policies will be emailed prior to your initial comprehensive evaluation. Some clients may require additional follow up to complete their initial evaluation, if they have an immense psychiatric history or there needs to be an extensive review of outside records. Subsequent visits will be medication management follow up. Please keep in mind when starting new medications, it may require the intervals between visits to be more frequent. This may require visits every two weeks initially, to monitor the safety and effectiveness of the medication and assess for side effects. The goal is to build a relationship of trust and help improve your wellbeing.
